Summary Care Records – Additional Information

 

As you may be aware, key parts of your medical record are shared with authorised healthcare staff when you need it the most – when you need unplanned care or when your GP surgery is closed. A Summary Care Record is created for every patient and contains information on your allergies & sensitivities, adverse reactions and current medication.

 

Summary Care Records are now being updated to include the following additional information:

Significant problems (past & present)

Significant procedures (past & present)

Anticipatory care information

End of Life information

Immunisations

This information is optional and access to this by healthcare staff outside of your GP surgery is controlled by you.
